Technical and Quality related Root Cause Analyses (RCA) and Incident Investigations are required to maintain and improve the quality of the products and services rendered, and for the enhancement of safety in the workplace.
RCA is a reactive method to identify both the obvious and the underlying causes of a non-conformance or incident so that specific solutions can be implemented.
A complete RCA consists of a clear definition of the problem, a thorough analysis supported by evidence, a specific corrective action plan for implementing the solutions and monitoring the effectiveness of those corrective actions.
For complex problems and costly blunders, a thoroughly documented multi-faceted incident investigation report may be required. A standardised layout, logical recording and well considered reasoning are of cardinal importance when the causes and contributing factors of incidents are to be established and presented.
The method of recording will not only make the job of the investigator(s) much easier but will also enable the Review Board to issue instructions that will effectively address the root cause(s) to prevent a future recurrence.
Why Should an Individual Attend?
The purpose of the course is to offer proven RCA and incident investigation techniques to enable course delegates to accurately establish the causal factors that led to a non-conformance or incident, and to recommend appropriate preventive and corrective actions.
Outcomes
On completion of this course, you will be able to:
Recognise that generally a chain of events is required to result in a non-conformance/incident.
Appreciate the significance of appropriate procedures and quality tools when investigating non-conformances and incidents.
Respect the importance of impartiality, thorough debating and diligent recording.
Undertake effective RCAs and compile meaningful incident investigation reports.

The School of Consulting Engineering was founded by CESA in 2000. The School aims to provide the opportunity for Consulting Engineers, their staff, their clients and other interested parties in the Built Environment to enhance their business, professional and entrepreneurial skills.
All training is interactive and participative. The methodology is designed to achieve maximum group participation. It allows delegates to take away real practical skills, rather than just theoretical knowledge.
The annual training programme comprises of seminars, workshops, short courses and skills programmes, which are developed to meet the specific requirements of the industry and are held all over the country.
If your company has more than 10 people wanting to attend a particular course, a dedicated in-house training course can be arranged at a time convenient to you. In addition to receiving a special group discount it also saves on travel time and is an excellent intellectual team building exercise, giving companies the ability to work with and discuss confidential company data.
Root Cause Analysis training (needs to have done Incident investigation) is offered by SHEQMAX to help you in getting to the root cause of the incident.
The short course in Total Quality Management will introduce you to contemporary tools and techniques in quality control that will enable you to articulate and implement quality improvement processes in the workplace in line with the total quality management philosophy.
ISO 9001: 2015 course is offered by IRCA Cape Town. We present a variety of SHEQ training courses. The courses incorporate international standards & practices and are easily adapted to the environment of the various companies we are working with.
ISO 9001 shows organizations how to manage and deliver quality products and services and meet your customer needs. This course is a comprehensive introduction to implementing a Quality Management System according to ISO 9001:2015 requirements.
The intention of this course is to provide delegates with the knowledge, understanding and the essential skills to perform Quality Management System (QMS) internal audits, against the ISO series of standards.
