Course Details

The West Toronto Diabetes Education Program helps improve the quality of life of adults living with type 2 diabetes, pre-diabetes, or those at risk of developing type 2 diabetes.

Registered nurses and dietitians facilitate individual and group counselling that provides information, resources and tools to help individuals self-manage their diabetes.

Topics covered include: understanding diabetes, medications, self monitoring blood sugars, healthy eating, reading labels, physical activity, stress management, sick day management, and emotional eating.

Group sessions offer participants tools and resources to support diabetes self management. The goal is for participants to:

  • Gain a broad understanding of diabetes, its complications and self management
  • Learn how to use a glucometer and interpret the results
  • Understand how foods, medications, stress and illness can affect blood sugars
  • Apply healthy eating and physical activity tips to your lifestyle

Who Is Eligible For Groups?

We encourage adults who have type 2 diabetes, pre-diabetes, or those at high risk for developing diabetes to attend. Group sessions are also open to the broad community and family members or caregivers are welcome.

Interpreters and tokens are available free of charge for individual and group counseling.
