Sherbourne Health is a dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and diverse communities to achieve wellness.

Our priority populations are

• Those living in the Downtown East Toronto neighbourhood

We are located in Moss Park, a diverse neighbourhood with a number of newcomers to Canada and a high number of people experiencing homelessness and precarious housing.

We are proud to provide services to people who may have trouble accessing health care.Through our Family Health Team and Integrated Health Services, we provide primary care. This approach to primary health care brings together different health care providers to coordinate your care.

Our Family Health Team consists of doctors, nurses, nurse practitioners, social workers, dietitians, and other health care professionals who work collaboratively, each using their experience and skills to provide care.

We are also dedicated to providing one-on-one psychotherapy with highly qualified staff who have expertise in our priority populations to build mental health in our communities.
