• Factors contributing to adverse incidents: Human factors, organisational factors, other contributory factors
• Which events should be investigated and by whom?
• Legal requirements for reporting accidents (RIDDOR).
• The principles and process of accident investigation.
• Information gathering and recording.
• Discussion of real-life scenarios.
