Course Details

The purpose of this course is to provide all persons who have occupational health and safety responsibilities with a basic understanding of how to identify the root causes(s) of incidents in order to take corrective action and what controls can be implemented to prevent further occurrences of such events.

Effective investigation, recording, and reporting of nonconformists are an important part of an effective SHE programmer.

Also to provide you with knowledge and skill to manage incidents, with specific emphasis on the identification of the root causes and applying the RCAM (root cause analysis method), taking corrective action and implementing control measures to prevent the reoccurrence of incidents.

You are not expected to have knowledge of the Occupational Health and Safety Act, Regulations or Standards before attending this Course.

    • Discussing the consequences of incidents
    • Examining Bird’s Ratio
    • Examining the loss causation model
  • Interpret the reporting and investigation of incidents in the workplace by:
    • Interpreting the reporting of workplace incidents
    • Analyzing the steps in the investigation process
  • Comprehend the Root Cause Analysis by:
    • Gaining an overview of Root Cause Analysis (RCA)
    • Discussing the general principles of RCA
    • Discussing the benefits of RCA
    • Understanding the steps in applying the RCA process
  • Analyse incidents in the workplace to determine the root cause/s by:
    • Applying the 5 Why analysis technique
    • Applying the Bowtie Technique
    • Applying the Incident Cause Analysis Method (ICAM)
