  • $50 Registration Fee

  • 2 copies of Canadian Citizen or Permanent Resident or Refugee (1 copy front side, 1 copy back side)

  • Proof of Quebec Residence: lease (page 1-3)

  • Renewal of lease if available

  • Hydro Statement (page 1-2 from January 2021 to May 2021)

  • CSQ

  • Medicare Card

  • Photocopy of Passport (1st page)
