Course Details

The main aim of this course is to provide the participants with an overview and basic knowledge of industrial incident investigation, thus enabling them to facilitate, lead, or contribute as an efficient team member in an incident investigation and analysis.

 

Intended Learning Outcome:

The participants will be able to address the necessary steps & precautions required in facts findings as well as perform causal analysis using Tripod Beta principles, report, and follow up on incidents by utilizing an established web-based management system.

 

Course Terminal Objectives:

On completion of the course, participants should be able to:

  • Describe the importance of incident investigation (knowledge);

  • Describe incident causation and contribution of human failures to an incident (knowledge);

  • To state and explain main elements in investigation procedures (knowledge);

  • Carrying out factual findings and interviews (skill);

  • Perform causal analysis by using Tripod Beta principles (skill);

  • Describe and explain the incident's notification & reporting mechanisms.

 

Target Groups:

All supervisory personnel who will be involved in an incident investigation in their respective workplaces.

 

Course Methodology:

  • This course will be presented in English. The course delivery format will be a combination of lectures, practicals, and team activities.

  • The measurement of learning shall be conducted through written tests (knowledge) and practical exercises (competence).

 

Pre-Requisite :

Working knowledge in the health, safety, and environment (HSE) and oil and gas industries is preferred.
